Facility will enroll staff in training regarding supervising children in the classroom. Facility will submit certificate/registration proof and a witten summary of understanding of the training to the Palmdale RO by due date by email. Based on the investigator’s investigation child#1 was injured due to not being properly supervised at the time of the incident staff #1 had her back to the children for an extended period of time.
