The facility agrees to submit an agenda of the material/topics that will be discussed with staff during a meeting that will be held the week of 06/29/26. Additionally, the facility will submit a list of all staff member's names who attended the meeting and signature. Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above. In reviewing staff files/records, LPA observed and determined that the TB test for staff #1 was taken in 2007 however, their hire date was in 2023.
