a evidence by Current assigned Director agrees to submit a Temporary Director Packet to the El Segundo Regional Office while the hiring process for a permanent Director is underway. Once a permanent Director is hired, a new Director packet will be submitted. Based on interviews and obsrvations the licensee did not comply with the section cited above in that the facility has not submitted a Director packet to the Department/there is no director which poses/posed a potential health, safety or personal rights risk to persons in care.

The Licensee will ensure that all ceiling tiles are in good repair and properly placed in their designated slots to ensure tiles are secure and flush with the ceiling grid. Photos documenting the completed repairs will be submitted to CCL by the due date of 07/09/2025. During a cite visit conducted at the facility on 7/2/25, at 11:30 AM, LPA observed several ceiling tiles in disrepair. The tiles were displaced from their designated slots in the ceiling. This poses a potential health, safety or personal rights risk to persons in care.
