  Director agreed to continue keeping the entrance door locked and allowing parents to enter and exit by means of the doorbell and facility staff. Director agreed to send LPA a copy of the repair information (invoice etc) from the technician once the repair is complete via email. Based on interviews, observation, and record review, tape was used to keep the entrance door open while the keypad was temporarily not working, which poses a potential health, safety, or personal rights risk to children in care.
