Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above. The facility does not have a working land-line. LPA arrived at the facility and was unable to make contact. LPA did a Google search and contacted director but was not at the facility. Licensee will demonstrating in writing how to meet licensing requirements standards and how these standards will be adhered to. LPA confirmed with licensee that the unusual incident occurred on 4/15/2022, this incident reported to licensing on 5/9/2022. This is poses a potential health and safety risk to children in care.
