Based on interviews and By COB 12/15/2025, Director stated they will submit a written statement on how facility will come back into compliance. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected. record review, the licensee did not comply with the section cited above when staff was observed on video aggresively handling a child by restraining a child with their arms and legs, pulling and grabbing child by the wrist which poses an immediate risk to the health, safety or personal rights of children in care. The licensee shall provide care and supervision as necessary to meet the children's needs.No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). To ensure there are no future incidents Site Supervisor shall provide staff training. Proof of staff training shall be submitted to CCL by 6/27/25. Proof that child's authorized representative was notified of the incident shall also be provided to CCL by 6/27/25. Supervision shall include visual observation.
