Facility Represenative states she will submit a copy of a memorandum to staff, which states the protocols on reporting injuries to authorized representatives and will submit a written statement of understanding of this regulation and a summary of CCL video on reporting and will submit copies to the department by email by 7/8/24. Based on interviews and record reviewed, the licensee did not comply in staff admitted in not notifying the authorized representative of a child’s injury at time of pick-up, which poses a potential Safety and Personal Rights risk to persons in care. ***This is an amended version of the report that was created on 7/1/24***

Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above in 31 out of 31 persons, as LPA observed a large wooden playhouse on the playground that had several areas of wood that was in disrepair which poses a potential health, safety or personal rights risk to persons in care. (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance The facility has made the sink that tested with an ALE out of service and posted appropriate signage. Sink is located in area used by adults only. Fixture(s) are to be retested by a certified water sampler and a follow-up water sample must be taken following the protocol. Fixture and piping has been replaced.
Based on record review, the licensee did not comply with the directive above, as small sink kitchen (water source C) used for staff hand washing tested with an Action Level Exceedance (ALE), which could pose a Health and Safety risk to children in care POC has been recived by the SDCCRO.
