Director stated a staff In-Service training will be scheduled by 11/21/25 to discuss the importance of abiding by personal rights of children in care, including scenarios and strategies for staff to implement. Based on interviews and S1's admission, the licensee did not comply with the section cited above in that it was determined on 11/7/25 S1 used inappropriate discipline to C1 and C1’s Personal Rights were violated which poses an immediate health and safety risk for persons in care. Director will submit a work order for repairs and submit a photo of correction to LPA by 10/28/2021. Based on LPA observation, staff did not ensure facilty playground was safe and in good repair. LPA observed, peeling paint, large splinters, protruding wire, and cracked and lifted cement in the 5 year old playground, which poses a potential health and safety risk to children in care.
