Deficient Practice Statement Based on observation and record review, the licensee did not comply with the section cited above. In conducting a physical inspection and in reviewing the facility sketch, LPA observed a middle classroom that was created by taking away space/square footage from the infant classroom. The Department did not receive updated documentation an/or authorized the alterations. This poses/posed a potential health, safety or personal rights risk to persons in care. In touring the facility playground's, LPAs observed gaps and cracks on the rubber matting that is underneath the play structures. The gaps and cracks create a tripping hazard.
