101212(c) · Reporting Requirements: The licensee shall notify the Department in writing of his/her intent prior to making any structural changes that reduce the total amount of indoor or outdoor activity space. Such structural changes shall include, but not be limited to, room additions. Licensee agrees to submit pictures of the infant crib area and updated facility sketch. To be submitted by 1/26/2022. This requirement was not being met as evidenced by observations and the licensee's own admission that the wall that separates the crib area and the activity area was moved on several occasions.
