During today's visit, preschool playground was inspected and found to be in substantial compliance. Director agrees to submit updated facility sketch to reflect all new playground changes and statement of estimate renovation completion. Update progress via LIC 624 to CCL. Per Director, Alisa Holtegarrd, center playground renovation/construction began 09/07/23; this will continue throughout the other playgrounds at the center one at a time until all play areas are completed. Based on observation and interviews, LPA's observed two children using the bathrooms unattended. The door to the bathrooms were closed when the children were inside the bathrooms, each child entering the bathroom by themselves, while the teacher stayed outside in the classroom. which posed a potential Health, Safety, and Personal Rights risk to the subject child and potential children in care Director stated a work order was submitted to remove the full doors in the preschool bathroom and install half doors. In the meantime, Director stated they will be making alterations to the existing door to make them into half doors. Director agrees to submit pictures to CCL once completed. Based on Interviews and records reviewed, on 08/04/22 Staff were transitioning between scheduled breaks, in this timeframe a child was struck multiple times and kicked by another child and as a result, sustained a fractured ankle before staff intervention. This poses an immediate health and safety risk to the children in care visual supervision and transitioning between scheduled breaks. In or about June 2021, there had been at least two different occasions that children were left unsupervised while staying in the front office area waiting for additional Current facility director Alisa Holtegarrd agreed to ensure children be provided with proper supervision at all times.
