The Acting Director shall create a plan detailing how they plan on preventing this type of incident from occurring again. This plan must be emailed to the LPA no later than 2/29/24. Based on staff interviews and records review it was determined that a child was able to leave the facility and go up the street without adult supervision; which poses an immediate risk to the health, safety, and Personal Rights of the children in care.
