Per Director, the staff will receive training on role and responsibility of Mandated Reporter and reporting requirements, proof of training and agenda will be sent to LPA by 2/9/26. Per Director, Child 1 reported incident which occured at the home to Staff 1 on 12/16/25. The incident was not reported to licensing until 1/13/26 (which exceeds the requirements to report within 24 hours), posing a potential risk to the health and safety of children in care.
