This requirement was no met as evidenced by Per Site Supervisor, the staff was reminded to take count after every transition, using name to face. The facility will conducted a training regarding active supervision. Agenda for training will be provided to LPA by 7/25/25

  Based on record review and interview the facility did not comply with the section cited above when Child 1 was found in outside playarea with no supervision.
