Based on LPA’s interviews, staff confirmed that an incident took place in May of 2022, where child #4 was injured while in care. Staff providing care and supervision disclosed being present however, statements from the staff who observed child #4 when child #4 was injured were inconsistent and neither staff present saw the incident occur. This poses a potential health and safety risk to children in care. Licensee submitted a declaration stating children will be visually supervised at all times to ensure children are safe. Licensee will take preventative measures prior to an incident occuring. License will contact R&R to take a training on managing challenging behaviors. Based on licensee's interview child #2 scratched child #1 during the time she was changing a diaper in the family room. The licensee did not observe the incident. This poses a potential risk to children in care
