Director will conduct training with staff covering the importance of 100% supervision. Director will provide the Department with documentation of the training criteria along with a list of the names of staff that participated in the training. Based on observation of surveillance video, C1 was left without supervision when they ran outside of the facility into the parking lot. This posed a potential risk to C1’s health, safety and personal rights. Director will also install additional secure devices to prevent children from opening the gate. Director is to develop and implement an individualized plan to address the personal needs and behaviors of chiildren.
