The Director shall ensure that awake infants are only cared for within approved indoor activity spaces and not in the designated sleeping area.Director shall retrain all infant staff on infant care regulations, including requirements for maintaining separate sleeping and activity areas. Based on interviews conducted, the facility allowed awake infants to use the sleeping area as an activity space violates the requirement that the infant sleeping area be physically separate from the indoor activity space, which poses a potential risk to the health, safety, or personal rights to children in care. Documentation of the staff training, including agenda and staff signatures, shall be submitted to the Department by 06/01/2026. LPA provided 101438.3 Indoor Activity Space for Infants regulations. by Per Director both employees were put on suspension on 07/17/2025 and terminated on 07/18/2025. Per director there will be a staff meeting and all employess will watch the Personal Rights Video on CCLD website. Director will submit a written agenda and sign in sheet by POC due date. Based on unusual incident report and staff interviews two employees engaged in a physical altercation while children were in care. This altercation took place in the hallway, all classroom doors were closed and children did not witness. Which poses a potential health, safety, or personal rights risk to persons in care.

No infant units were provided as evidence of qualification. which poses a potential health, safety, or personal rights risk to persons in care classroom so that compliance is maintained. Assistant Director stated she will submit a plan to the Fresno Regional Office by 02/07/2025 specifying how staff will be reorganized to ensure there is a qualified teacher in the infant classroom at all times.
