Per Program Director, Erin Wilcox, staff received training on Personal Rights on 11/12/2025. Program Director will provide an updated protocol on how staff will ensure children’s personal rights are not infringed upon.

It is determined that Staff #1 threated Child and or chidren verbally. This poses a potential risk to the health and safety, or personal rights of children in care Protocol shall include a statement on how program director will provide unannounced observation of staff when they are working with children in care. This will be provided to the Fresno South Regional office by 12/23/2025.

Per Guardian Employee Roster, 2 adults were not cleared with fingerprints and were not associated with the facility, which poses a potential health, safety or personal rights risk to children in care children are fingerprint cleared and are associated with this facility. evidenced by Licensee agrees to replace all child safety gates and provide photos and receipts to LPA by August 15, 2025. Licensee also agrees to Submit a plan of how to mitigate the child safety gate issue for a long term solution. Based on observation LPA and LPM observed a unserviceable child safety gate which allowed C1 to leave the classroom unobstructed which poses a potential Safety or Personal Rights risk to children in care.

evidenced by Licensee agrees to review proper handling of children by reviewing personal rights in CCR and conduct training with all the staff and provide proof to LPA by August 20, 2025 and a written letter by S1 on what could have been done different pertaining to the situation. Based on observation, LPM and LPA observed S1 pull C1’s arm at an elevated angle with enough force to slightly elevate the child off the ground. LPM Herring immediately and tactfully intervened, instructing S1 to release the child and addressing the inappropriate handling of the situation which poses a potential Safety or Personal Rights risk to children in care. Licensee requested and agreed to participate in TSP. They will also train facility staff on proper supervision. They will submit a written report on what the training will be on by POC date. that confirm that child 1 walked out of the facility unsupervised on or around March or April 2024. This poses an immediate risk to the health and safety or personal rights of children. Once the training takes place they will submit a sign in sheet of who atteneded the training.
