Based on interviews and record reviews, staff left a child unsupervised without the required adult supervision Corrections for all deficiencies were received at the Fresno Regional Office before February 27,2026. Facility did provide in house training on care and supervision. The facility has been recommended to the Technical Support Program (TSP) for assistance. Child Development Site Supervisor II, Rene Orender-Maston and Executive Director, Lisa Vargas have agreed to participate. No violations were cited during today’s meeting. It was discussed that continued violations of Title 22 Regulations and failure to maintain compliance will result in a Non-Compliance conference and may be referred to our Legal Division for possible Administrative Action. A copy of this signed report was given to Child Development Site Supervisor II, Rene Orender-Maston and Executive Director, Lisa Vargas . by Per Site Supervisor II, an active supervision training will be conducted on 2/27/2026. Site Supervisor II will provide a sign-in sheet of attendees and training material to Fresno Regional Office by 3/5/2026. This inicident was also self reported to CCLD. Based on interviews and record reviews, it was revealed that staff left child unsupervised from multiple minutes which is an immediate risk to the personal rights, health, and safety to the children in care.
