Based on interviews conducted and records obtained during the Complaint Report investigation completed by LPA Paul Garcia with the Department of Social Services Community Care Licensing Division, it was discovered that Director Sara Vinson permitted child #1 (C1) to consistently hurt many of the community’s children in the preschool over several years that resulted in insufficient protection to children related to enhanced child-on-child aggression.

Based on interviews and documents obtained the licensee failed to follow their own Discontinuation of Services Policy in their Plan of Operation pertaining to protecting children or persons at the facility Personnel Requirements On August 13, 2024, a Case Management report documented that Canyon Hills Preschool Shall be required to provide an additional staff member that shall not be included in the center's staffing plan to solely supervise child #1 (C1) at all times and Canyon Hills Preschool shall also provide an additional staff member to solely supervise any child exhibiting reasonable similarities of child-on-child aggression with similar frequencies remotely resembling C1s documented conduct. Corrections for all deficiencies were received at the Fresno Regional Office before August 29, 2024. Per Directors request, the facility has been referred to the Technical Support Program (TSP) for assistance. It was discussed that continued violations of Title 22 Regulations and failure to maintain compliance will result in a Non-Compliance conference and may be referred to our Legal Division for possible Administrative Action. Based on interviews and documents obtained the licensee Licensee agrees to review the reporting requirements training on the CCL website and submit a full two (2) page hand written statement of what those requirements are and provide relevant examples of occurrences that must be reported. Written statement shall be mailed to CCLD office did not report numerous Unusual Incident Reports to the Fresno Community Care Licensing Division as required per a condition of the license which poses a potential Health and Safety and/or, Personal Rights risk to persons in care. no later than August 29, 2024. The Administrator agrees to implement a plan and training for all staff that shall write a detailed declaration for all Unusual Incident Reports and Opportunity for Improvement Reports for children demonstrating extreme behavioral issues.

Licensee was informed by LPA and letter that she and the entire Board of Directors shall attend an informal meeting on August 30, 2024, at the Fresno Regional Child Care Office. Licensee was informed that she SHALL attend an Informal meeting at the Fresno Based upon information revealed through interviews and records collected, it was discovered that Director Sara Vinson has permitted child one (1) to consistently hurt many of the community’s children in the preschool over several years that resulted in insufficient protection to children related to child-on-child aggression. Regional Child Care Office on August 30, 2024, with ALL Board of Directors to be present. Failure to do so may be considered a failure to abide by the terms and conditions of the childcare license issued by CCLD. This facility will also be referred to our Technical Support Program (TSP).

by Assistant Director, Crystal Mallard stated her staff will hold a mandatory staff training and implement a system that would eliminate occurrences of this nature in the future. Based on document review and Interview with a witnesses, it was determined that a child to remained in a wet pull-up diaper. This poses a potential risk to the health, safety, and personal rights of children in care. Assistant Director, Crystal Mallard stated she will provide CCL with an outline of the system covered as well as a log of staff who attend the training by 11/15/2023.

101700.3(b)(1) · (a) California’s Action Level for lead in water at Child Care Centers is 5 ppb. (b) Testing results with fractional ppb readings of 0.5 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. Testing results with fractional ppb readings of less than 0.5 ppb shall be rounded down to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. (2) Licensees shall maintain a lead value at or below the Action Level of 5 ppb in all outlets subject to the testing requirements of these Written Directives, for the health and safety of children in care. (c) If testing indicates an Action Level Exceedance at any water outlet, the water from that outlet is deemed not safe to drink and an immediate response pursuant to section 101704 shall be required. Director agrees to submit evidence of replacement, removal, and/or remediation of outlet I, in Classroom #6, to the Fresno Community Care Licensing Office 02/24/23. Director agrees to notify parents, guardians, and/or authorized representatives of results, and post (ALE) results in a reasonable location as soon as possible.
