    based on observation, the The school age program does not have exclusive use of the outdoor activity space and the existing waiver needs to be modified. The Director agrees to provide the Department with updated outdoor play schedule for the school age license and preschool license along with a waiver facility allowed school-age center children on the outdoor activity space with preschool children which poses a potential health, safety or personal rights risk to persons in care. request for a shared playground use by 07/16/2025. The Director will no longer allow preschool children and school age children to use the outdoor activity space at the same time.     Based on observation, there was not enough cushioning material The Director agrees to order enough cushioning material to be in compliance with the manufacturer’s instructions for the school age play structure. The Director will send LPA proof of purchase by 07/07/2025. The Director will make the play structure off limits to all children until the cushioning material arrives. to absorb falls near high climbing equipment. Cracks in the rubber coating and chipped paint on the climbing ladders were also observed during the inspection which poses an an immediate health, safety or personal rights risk to persons in care.
