Deficient Practice Statement Based on observation and record review, the licensee did not comply with the section cited above. LPA observed that water used for drinking and food preparation is provided through a water filtration system that is not separate from the building. The facility was unable to provide documentation showing the facility was tested for lead which poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 08/26/2025
