Per Site Supervisor, a work order will be put in to request repair/replacement for all Play Yard cushioned material. Until completion, children will stagger the use of the play yards not being worked on. Copy of work order, response, and plan will be submitted by POC date. Based on Interview and Observation, Licensee did not ensure that the play yard cushioned material was in safe condition for the children in care, resulting in Child #1 sustaining a head injury, needing medical evaluation. This poses a potential risk to children's health, safety, & personal rights. Plan should include: start date, yards availble for play, during what part of day/week work will be occurring, and approximate end date.
