    Site supervisor agrees to submit proof to the Department that staff training will be conducted on the topic of playground supervision and safety, with a focus on the proper use of the slide for children in care. Site supervisor will submit proof of the play structure manufacturer’s information, along with the training topics and attendance, to the Department by 04/02/2026. Based on the record review and interviews conducted, it was determined that staff failed to ensure that C1’s personal rights were protected by not ensuring C1’s safety. C1 should not have been allowed to climb up from the bottom of the slide, which resulted in C1 losing balance, falling, and sustaining an injury. This failure poses a potential risk to the health, safety, and personal rights of children in care. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. The Education Coordinator was informed of the childcare training videos available on the Community Care Licensing (CCL) website at www.ccld.childcarevideos.org. The Education Coordinator and staff will view the following trainings: Supervising Children in Child Care Centers, Children’s Personal Rights in Child Care, and Health and Safety Training. By 01/12/2026, the facility will submit copies of staff attendance sign in sheets, including dates of completion, to verify that all staff members have received training and understand children’s supervision requirements, children’s personal rights, and health and safety practices for children in care.

    Based on the record review and interviews conducted, it was confirmed that C1 was left unsupervised in the playground for seven minutes without adult supervision. During this inspection, Site Supervisor provided documentation proof that an immediate training pertaining to active supervision was completed with all staff members on 10/28/2025, followed by an all staff meeting on 10/31/2025 discussing key success to “Communication and Teamwork” as teamwork needed in the classroom.

    Based on record review and interviews conducted, it was confirmed that due to the lack of supervision, Child #1 (C1) had access to the off-limits. This poses a potential risk to the health, safety, or personal rights of children in care. Site Supervisor stated that she will also continue to monitor all staff members and enforce these training materials in the classroom.     Per Director, she agrees that she will conduct a training on children’s personal rights and training regarding the appropriate protocol to take if a child is extremely ill. Director is to submit proof of attendance and protocol training materials to CCL-Fresno Office by 02/29/2024. Based on record review and interviews conducted, C1 started having fever at 8:20 AM at 100.9, at 12:30 PM, C1’s fever was 102.4 and by 1:15 PM, C1 fever was at 103.2. During the interviews, staff were not aware of the protocol if a child involved is extremely ill and if the child’s parent is not able to be reached. Staff were also not able to identify the next appropriate protocol to take. This poses an immediate risk to the health, safety, or personal rights of children in care.
