interview and record review. Multiple interviews revealed that staff have Licensee Puga stated she will meet with staff and review all Personal Rights requirements with staff. Licensee stated she will actively monitor her staff to ensure that all personal rights requirements are being adhered to. Written meeting agenda with staff signatures will be provided to the Department by POC date 5/6/2022. yelled in an aggressive tone to address children in care and that child #2 caused open bite marks on child #1. During the inspection of restroom #3, it was observed that the floor was dirty and Licensee Puga stated she would send in a proof of cleaned floors and sink and stated she would montior all restrooms in her facility. Proof will be submitted by POC date 11/5/2021. the sink was discolored and yellowing in some areas. This poses a potential risk to the health, safety and or personal rights of children in care. observation and interview Director was observed to move to the other side of the solid partitions to assist staff #1 placing them back in ratio. Director or Licensee will submit a wrriten plan to ensure the facility is following ratio requirements at all times. During circle time, there was one teacher (staff 1) supervising 19 children, while 3 other staff were on the other side of the room and could not provide visual observation.
