101229(a)(1) · Responsibility of Providing Care and Supervision - No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. The staff member (S-1) involved was given a written warning. S-1 attended training on active supervision on 04/20/23. Copy of written warning, training agenda and proof of attendance were submitted during the inspection. Deficiency corrected.

On 03/01/23, a child (C-1) was left unattended outside Baskin 2 Room. This poses an immediate risk to the health, safety and personal rights of children in care Since this is a zero tolerance violation, an immediate civil penalty of $500 was assessed.
