Director stated an awning was ordered and the shade will be installed. Director stated she will email LPA the work order for the awning and a proof the the shade will be installed by due date of 6/24/24 Based on observations and interviews, four (4) staff stated the shade in the infant yard broke a few months ago leaving the area without shade This poses as a potential risk to health and safety of children in care.

Director stated the rugs will not be put back in and turf grass will be installed. A document stating of the sanitation process will be sent to LPA by due date of 6/24/24. LPA observed there were no more rugs in the outdoor play area Based on observation, the rugs and mat in the outdoor area were damp and full of dirt and insects came crawling out of the rugs when inspected. There was a bucket with stagnant brownish water where the toys were.This poses as a potential risk to health and safety of children in care.
