102417(g)(9)(A) · Operation of A Family Child Care Home (g) The home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not limited to: (9) Each family child care home shall have a written disaster plan of action prepared on a form approved by the Department. All children, age and ability permitting, and the provider, the assistant provider, and other members of the household, shall be instructed in their duties under the disaster plan. As their age and ability permit, newly enrolled children shall be informed promptly of their duties as required in the plan. (A) Each family child care home shall conduct fire drills and disaster drills at least once every six months.

102425(j)(5)(A) · 102425(j)(5)(A) Infant Safe Sleep, If the infant is sleeping in a separate room from where the provider is stationed, the door to the room the infant is sleeping in shall remain open at all times. (A) The provider shall be able to visually observe the infant without moving the door. This regulation was not met The licensee stated she will keep the door open to ensure she can visually supervise the child. The licensee will send a declaration to the LPA by the due date.

The licensee removed the saucer chair and rocking chair from the daycare area. The licensee stated she would return the saucer chair and the stop using the rocking chair for daycare children. 102425(c) · Infant Safe Sleep 102425(c) An Individual Infant Sleeping Plan [LIC 9227 (3/20)] shall be completed for each infant up to 12 month of age the provider has in care and maintained at the facility in the infant’s file.
LPA Odom provided a copy of LIC9227 form and explained each section. Licensee will have C1 parent's complete the form and place in child's file. Licensee will take a picture and send to LPA Odom for proof of completion by 8/18/21. Based on children file reviews at 10:00am C1 did not have the Safe sleep plan LIC9227 in the file. Licensee stated, I am not familiar with the Safe Sleep Plan form for infant 0-12 months. This poses a potential health and safety to the children in care.

LPA Odom explained how to create a napping log and what needs to be documented on the log daily. Licensee will create a napping log for each infant child and send a copy to the department by 8/18/21. Based on observation and file reviews at 10:00am, Licensee did not have a sleeping 15-minute log documenting for labor breathing, signs of distress, infant movement and visually observing 15 minutes.
