The director stated she will document the services provided to infants to provide parents a copy and keep a copy in the facility. The director will send evidnce of the documentation to the LPA by 11/20/24. Based on staff interviews. S3 disclosed he/she observed C1 scratching and applied ointment and left work before noon leaving the child under the supervision of staff who did not meet C1’s needs. The child incurred injuries. 101170(e) · 101170(e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility… The licensee did not meet this requirement as evidenced by Staff will exit the facility and obtain a clearance or transfer any existing clearance to the facility. The staff will not return until cleared and associated to the facility. The director will send a request for clearance or clearance trasnsfer within 24 hours. Based on records reviewed and interviews conducted, the regulation was not met. 101430(a)(3)(A)(4)(a) · Infant Care Activities (A) Staff shall place infants up to 12-months of age on their backs for sleeping. (4) Infants with an Individual Infant Sleeping Plan [LIC 9227 (3/20)] that have Section C of the form completed and signed by an authorized representative shall be placed on their back when first laid down to sleep. In the event the infant changes position, the infant may remain in the alternative position. (a) Upon staff observation that the infant is capable of rolling from their back to their stomach and stomach to their back, the provider shall fill out Section D of the Individual Infant Sleeping Plan [LIC 9227 (3/20)], notify the authorized representative, and obtain the authorized representative's signature on the individual sleeping plan no later than the next business day following observation.

Director stated that she will have an a training with the staff and establish a protocol so that changes in infant's feeding plans are communicated to the Director in order to update the plan in writing. The director will provide materials and attendance sheet. Based on records and interview statements, the individual infant feeding plan was not updated to reflect the changes in the C1’s dietary plan, which poses/posed an immediate risk to the health of the children in care.

Director stated she will hold a training with staff to discuss children accomodations and provide evidence of attendance and materials. Based on records and interview statements, staff stated that C1 got a hold of restricted food that caused C1 diarrhea on two or more occasions.
