The purpose of the inspection was to conduct a Complaint investigation for case #00120917 on March 5, 2025. One fingerprint clearance card was verified to be valid through the DPS website during the time of the inspection. Ratios observed were: Children : Staff Infants- 13 : 3 Ones- 11 : 2 Twos- 11 : 2 Threes- 14 : 2 Threes, Fours, and Fives-12 : 1 There were 4 staff members interviewed during this investigation. There was 1 staff file reviewed during this investigation. There was 1 child’s file reviewed during this investigation. Others interviewed: Complainant Documentation observed was: Hospital After Visit Summary Upon completion of the Complaint investigation, it was determined from staff interviews, and documentation that two of two allegations were substantiated. The following deficiencies were observed and are subject to changes pending programmatic review.
