  The following deficiency was found at the time of the Mid-year Inspection conducted on 2/13/2026, and is subject to changes pending programmatic review. The Compliance Officer provided the Group Home with a paper copy of the Notice of Inspection Rights at the start of the inspection. Note: A full inspection was not conducted. 3 of 3 fingerprint clearance cards were verified to be valid on the DPS website during the time of the inspection.

The purpose of the inspection was to conduct a Complaint Investigation for case # 00157157 and # 00157173 on 2/13/2026. A full inspection was not conducted at this time. The Compliance Officer provided the Group Home with a paper copy of the Notice of Inspection Rights at the start of the inspection. Ratios observed were: 2:2 There were 2 staff members interviewed during this investigation. There were 2 staff files reviewed during this investigation. Upon completion of the complaint investigation, it was determined from observations, interviews, and documentation that the allegation was not able to be substantiated. 3 of 3 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection.
