The following deficiencies were observed at the time of the Complaint # 169259 investigation conducted on 5/13/2026, subject to changes pending programmatic review. A focused inspection was conducted. The following room ratios were observed: Infants: 1:4 One's: 1:5 One's/Two's: 2:8 Two's/Three's: 1:7 Three's/Four's: 2:11 Four's: 1:7 Three staff members were interviewed during this investigation. Upon completion of the Complaint investigation, it was determined from observation and staff interview that 2 of 2 allegations were substantiated. The following citations were observed.

The Compliance Officer spoke with the Complainant on 1/23/2026. A full inspection was not conducted. The following room ratios were observed: Infants: 1:3 One's: 2:10; 1:6 Two's: 2:7 Three's: 1:10 Four's: 1:5 Three staff members were interviewed during this investigation. 1 staff file was reviewed at the time of the inspection. 1 of 1 Fingerprint clearance cards reviewed was valid via a DPS website search. The following documentation was reviewed at the time of the inspection: Incident and injury reports, pictures, and staff statements. Upon completion of the Complaint investigation, it was determined from observation and staff interview that 1 of 1 allegations lacked sufficient evidence to be substantiated.
