The purpose of the inspection was to conduct complaint #00147359 investigation on 10/10/25. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. Ratios observed were: 2:6 (3 & 4 year olds). Both of the group home staff were interviewed during this investigation. One child was interviewed. Others interviewed: The Compliance Officer attempted to contact the complainant but the complainant did not contact the CO back. Documentation observed was the following: *Rosters, staff sign-in, children's attendance logs and 1 staff file. The fingerprint clearance card for the staff was verified to be valid through the DPS website. Upon completion of the complaint investigation, it was determined from observation, interview and documentation that the allegation was substantiated. The following deficiencies were observed and are subject to changes pending programmatic review.
