  Tummy time rules were discussed. There were 5 staff files reviewed. 5 of the 5 fingerprint clearance cards were verified to be valid through the DPS website.

The purpose of the inspection was to conduct complaint #00158980 investigation on 2/18/2026. A full inspection was not conducted at this time. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. Mandated reporting requirements were also discussed at this time. Ratios observed were:; One-two year olds: 1:4 Three-five year olds: 2:9. There was 1 staff interviewed during this investigation. There were 2 children interviewed during this investigation. Others interviewed: The complainant's contact information was not provided on the complaint form and the complainant was not able to be contacted for an interview. Documentation observed was email correspondence regarding this incident. Upon completion of the complaint investigation, it was determined from observation, interview and documentation that the 6 allegations lacked sufficient evidence and were unable to be substantiated. The following deficiency was observed and are subject to changes pending programmatic review.
