There were no deficiencies observed at the time of the Complaint # 164494 investigation conducted on 4/9/2026, subject to changes pending programmatic review. The Compliance Officer attempted to contact the Complainant. A full inspection was not conducted. The following room ratios were observed: One's: 3:17 Two's: 3:19; 3:17 Three's: 2:15; 2:16 Four's: 1:11; 2:15 Four staff members were interviewed during this investigation. The following documentation was reviewed at the time of the inspection: Incident and injury reports and video footage. Upon completion of the Complaint investigation, it was determined from observation and staff interview that 1 of 1 allegations lacked sufficient evidence to be substantiated.
