The following deficiencies were observed at the time of the Complaint Investigation (case 00132576) conducted on 06/13/2025 and are subject to changes pending programmatic review. The Plan of Corrections will not be accepted at this time. The Notice of Inspection Rights were provided to the licensee at the time of the inspection. The following staff-to-children ratio was observed: Classroom 1, ages 3-10: 2:16 The Department contacted the complainant on 06/03/2025. The Director and Licensee were interviewed. Documentation reviewed: 1 staff file 1 non-staff member's fingerprint clearance card. Staff attendance records for May and June 2025. Children attendance records for May and June 2025. Classroom rosters for May and June 2025. Children's Emergency, Information, and, Immunization Record cards. The fingerprint clearance cards for 1 staff member and 1 non-staff member were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: Individualized plan Special needs diapering Upon completion of the Complaint Investigation (case 00132576), it was determined from facility documentation, Director/Licensee's interviews, and the Compliance Officers' observations that 3 of 3 allegations were substantiated.
