Others interviewed: The Compliance Officer was not able to contact the complainant due to a lack of viable contact information. Documentation reviewed: Incident report, rosters. Upon completion of the complaint investigation # 00168303, it was determined from observation, interview and documentation, that 1 of 1 allegation was substantiated.

Documentation reviewed: Incident reports, attendance records, rosters, and facility video footage. The Compliance Officer contacted the complainant via telephone and email on 1/12/26 and 1/13/26. Upon completion of the complaint investigation # 00156100, it was determined from observation, interview and documentation, that 1 of 1 allegation was unable to be substantiated.
