The purpose of the inspection was to conduct a complaint investigation for complaint #166893 on 04/22/2026. A full inspection was not conducted at this time. The Notice of Inspection Rights were provided to the Licensee at the time of the Inspection. Ratios observed were: Infants: 1:4 1-yr-old Children: 2:10 2-yr-old Children: 1:8 2-yr-old Children: 2:12 3-yr-old Children: 1:13 3-yr-old Children: 2:16 4-yr-old Children: 2:21 4-yr-old Children: 2:22 There were 3 Staff interviewed during this investigation. Others interviewed: Director, Assistant Director Documentation observed was: One staff file, Child Attendance records, Child Information, Information, and Immunization Record cards. Upon completion of the complaint investigation, it was determined from observations, interviews, and documentation that the 1:3 allegations was able to be substantiated. The following deficiencies were observed and are subject to changes pending programmatic review. -Patting of child’s back during nap time. -Tone of voice while re-directing children.

A telephone call was made to the Complainant on 3/9/26. An email message was sent to the Complainant on 3/9/26. One staff file was reviewed during this investigation. The following documentation was reviewed: Classroom rosters. Upon completion of the complaint investigation, it was determined from observation, staff interview and documentation that 1 of 3 allegations was substantiated. The other 2 allegations lacked sufficient evidence to be substantiated. The following deficiencies were observed.

The Compliance Officer spoke with the Complainant on 2/19/2026. A full inspection was not conducted. The following room ratios were observed: Infant's: 2:8 One's: 1:6; 1:6 Two's: 1:8; 1:8 Three's: 1:13; 2:20 Four's: 2:21; 2:15 Four staff members were interviewed during this investigation. 1 staff file was reviewed as part of the investigation. 1 of 1 Fingerprint clearance cards reviewed were valid via a DPS website search. Upon completion of the Complaint investigation, it was determined from observation and staff interview that 1 of 3 allegations was substantiated.
