The purpose of the inspection was to conduct complaint case #000136149 investigation on 7/15/25 A full inspection was not conducted at this time. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. Ratios observed were: Infants: 1:3; One-year-olds: 1:2; Two-three-year-olds: 1:4; Three-School age:1:4 and 1:4. There were 6 staff interviewed during this investigation. Others interviewed: The Compliance Officer attempted to contact the complainant twice and the Complainant did not respond. Documentation observed were rosters. Four staff files were reviewed and the fingerprint clearance cards for 3 of the 4 staff were verified to be valid per the DPS website. Note: One of the staff is not an adult and is not required to have a fingerprint clearance card. Upon completion of the complaint investigation, it was determined from observation, interview and documentation that allegation #1 was substantiated. Allegations #2, #3 and #4 lacked sufficient evidence and were unable to be substantiated. The following was discussed, but not limited to: 1) Side-by-side occupied cribs are required to be spaced 24 inches apart if the crib does not have a non-porous barrier ; 2) Mandated reporting requirements; and 3) Staff files are required to be maintained on premises (either electronically or a paper file) if staff from the other location is working at this location. The following deficiencies were observed and are subject to changes pending programmatic review.
