The purpose of the investigation was to conduct complaint #00146768 investigation on 10/17/2025. A full inspection was not conducted at this time. Ratios observed were: Infants: 1:4 1-year-old Children: 2:4 2-year-old Children: 1:3 3-year-old Children and 4-year-old Children: 2:8 There were 3 staff members interviewed during this investigation. Documentation observed was Parent Handbook, Toilet Training packet and agreement, Admission and release records, daily information through Procare App. Upon completion of the complaint investigation, it was determined from observation, interview, and documentation that 3 of 3 allegations lacked sufficient evident and were unable to be substantiated. There were no deficiencies observed at the time of the inspection and are subject to changes pending programmatic review.
