The purpose of the inspection was to conduct complaint #00173813 investigation on 06/30/2026. A full inspection was not conducted at this time. Ratios observed were: 4 and 5-year-old children: 1:4 There were 2 staff members interviewed during this investigation. Documentation observed was: 3 Staff Files Upon completion of the complaint investigation, it was determined from observations, interviews, and documentation that two of the two allegations for complaint #00173813 were unable to be substantiated due to lack of evidence. No deficiencies were observed and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies.
