A clear bottle of hand sanitizer located on the sink of the main hallway near the older three -year-old classroom was accessible to children. This was corrected when the director placed the hand sanitizer in a locked closet out of reach of children.

A child in care at the time of inspection did not have a current immunization record on file as the MMR and HepA vaccines were not up to date. This was corrected at inspection when the director was able to provide the updated immunization list from the provider.

It was observed that the operational policies did not include statements in regard to the contact information for Child Care Regulation under Health and Human Services as the information listed was for the Department of Family and Protective Services. In addition, there were no statements regarding Infant safe sleep, hearing and vision screening, water activities, promotion of indoor and outdoor activities, insect repellent, sunscreen, epinephrine/medications and inclusive care in regard to special needs children in accordance to the updated minimum standards requirements.

There were a total of 9 infant children present at the time of inspection without current feeding instructions on file that had been signed by a parent/guardian within 30 days. Three of the feeding instructions were corrected during inspection when the parents updated the forms.
