Room C, the toddler classroom did not have the mats/cots labeled within the classroom. A total of 12 children were observed on mats that were not labeled, and/or labeled incorrectly. The operation will utilize a number chart to specify which cot/mat belongs to which child. The numbers were added to cots while Licensing was present.

The caregiver observed was not following the provisional status, which states that the individual could not care for children alone, or unsupervised. One caregiver was added to the classroom in order to abide by the provisional status.
