There was a small rebar found on one of the playgrounds accessible to children. This was CORRECTED when the rebar was removed from the playground. The 4 year old playground had an unsecured gate that led out to a busy road. This was CORRECTED when the director secured the latch by placing a bungy cord on it. There was a jump rope found wrapped around the base of a play equipment on the 3 year old playground that presented an entanglement hazard. This was CORRECTED when the director cut the jump rope.

The caregivers were observed in two infant rooms not washing the infants hands after diaper changing. This was CORRECTED when the caregivers were explained about the standard of washing an infants hands.
