The outdoor fence has protruding nails and there are screws and utility items exposed and accessible to children. Glass door that leads to the infant room its not marked at the children's eye level.

Toddler / Prek and School-Age rooms were observed with accumulated dust / dirt on the floors, walls/furniture, ceiling fans and parts of the side molding are out of place exposing the wall material.

(2) Communication, including: (2)(A) The emergency telephone number that is on file with us; and (2)(B) How you will communicate with local authorities; and (3) How your staff will evacuate and relocate with the essential documentation including: (3)(A) Parent and emergency contact telephone numbers for each child in care; (3)(B) Authorization for emergency care for each child in care; and (3)(C) The child tracking system information for children in care; (4) How your staff will continue to care for the children until each child has been released; and (5) How you will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted.
