Operational Policies were reviewed and are missing the following items: (10) Nutrition - statement indicating that if parent provides meals/snacks, parent understands the child-care center is not responsible for its nutritional value or for meeting the child's daily food needs; (18)(f) The criteria you will use to determine when extreme weather conditions pose a significant health risk that prohibits or limits outdoor play; and (18)(G) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play; (19) Procedures for providing and applying insect repellant and sunscreen.

Operation has a play structure with a height of 6ft. The structure is surrounded by 5-6 inches of tire mulch.
