    1/The playground equipment for toddlers was rotted and there were some sharp edges on it posing hazard for outdoor activities. Note: The second director put up yellow tape so children would not use it until it fixed. 2/There was one nail on the fence sticked out posing hazard for outdoor activities. Note: The first director fixed it by hammer at inspection. 3/One exit fence door from the playground could not be opened by 2 teachers as required that the children must be able to safely exit the building within three minutes in case of emergency. Note: The second director could open it after a while and stated that she will fix the fence hinge of the exit door from the playground so it can be opened in case of emergency. 4/The 4 seats of the play equipment of seesaw were cracked/broken posing hazard for outdoor activities.     There was a broken fence board in the outdoor play area.There were also a pile of nails along side the fence. In the play area a gate leading toward the front parking lot had a broken latch leaving the gate ajar and accessible to children. Note: The fence board was repaired, the pile of nails were removed and the gate was repaired and was able to close all the way. These repairs were made during the inspection.
