The outside play area had chicken wire that was detached from the frame which exposed rigid sharp edges. There were also three wooden palettes stacked against the fence which had split wood and protruding rusted nails.

The Emergency Preparedness Plan did not have a statement regarding how the provider will evacuate and relocate with the essential documentation including: (A) Parent and emergency contact telephone numbers for each child in care; (B) Authorization for emergency care for each child in care; and (C) The attendance record information for children in care at the time of the emergency.
