A 5 month old infant was sleeping in a bouncer inside her assigned crib. The daycare has been informed by the parent of a medically necessary exception for sleep but the sleep exception form has not been provided by the healthcare professional. Note: During the inspection the doctor's office was contacted for the needed document. The child was moved from the bouncer. Infant will sleep in crib until document is obtained.
