1.A TV that was mounted on the wall had an extension cord which was accessible to the children in care. The cord could be pulled, making the equipment fall on a child. The cords must be made inaccessible to the children in care. 2.The fence, around the outdoor playground, was observed to have an area that was not attached to the ground.
